Appointments

Upon referral by your dentist, you may contact our North York location at (416) 490-9998 or our Stouffville location at (905) 640-0161. One of our friendly administrative assistants shall answer your inquiries. You will be asked for information such as:

1. Referring dentist;


2. Tooth requiring root canal therapy or consultation;


3. Time of appointment;


4. Location of appointment (North York vs. Stouffville);


5. Nature of appointment (Consult vs. Treatment);


6. Insurance Information where applicable.

You will be informed about:

1. Our professional fees in accordance with Specialist's guidelines;


2. Directions to our clinics and parking;


3. What to bring for your first visit

First Visit

Upon arrival, you shall be greeted by our office manager. Please submit all relevant records including your referral, previous x-rays, reports, or anything you feel that is pertinent to our office manager. She will then assist you to complete our general Health Record. If you wish to fill out this record beforehand, please click here for PDF File. (PDF File: Patient Health Record). You will also be asked to complete our Consent Form for purpose of confidentiality and privacy. (PDF File: Patient Consent Form).

If your first language is not English and have difficulties in completing these forms, we have members on staff who are fluent in Cantonese and Mandarin.
